I've scanned through this thread and the Forum Guitars page on Toonz.ca but I still have two questions, hopefully they're not both dumb questions.
What happened between 2012 where the Toonz page says " March 2012 — never built " and 2021 where they are on order?
Is there still any possibility of getting one?  I realize this may be a LongShot but I have to ask.
1. Discussion about the current F-VI is at:
http://www.larriveeforum.com/smf/index.php?topic=54564.0
It was agreed that only actual "production" forum guitars would use an actual "forum XX" number.
Since the older Forum VI effort (a completely different guitar) did not go into production (did not meet the minimum order quantity), we used the F-VI designation for the current effort.
2. It is way too late to order a F-VI. Ordering was closed over 3 months ago.

I thought I remembered that as well. That he was building a couple extras.

Update
I sent an inquiry.....
John says the bodies are matched up and in the queue for side bending, after which things start flowing.
John will let us know right away when we hit that stage.
I didn't pursue how long the queue might be, though I have mental images.
The shortest queue was when I went to Texas DMV offices in Pleasanton a few years ago. One person was ahead of me.
The longest was going to Systembolagent (Swedish state liquor stores) in Stockholm a few days before Christmas, back in 1996. Whoa.

Yes, John said awhile back that 2 extra LS and 2 extra LSV would be made in case of production difficulties.
That was before our final order number reached 15.
Even earlier on, I believe as many as 20 good sets for the walnut had been set aside, but I can't quote a reference.

Now and then, I am contacted about the "waiting list", and then after some friendly conversation, these folks get referred to John, since being on the waiting list is sort of the same as an order commitment. How many are on the list I don't know. I also somehow maybe sort of got the possible understanding that Larrivee may reserve one of the extras for some kind of Larrivee Forum promotion. John is fixated on this forum and it's success.

Great news,     I could use it after getting t-boned last week and my less than 6 month old car getting written off, the other driver being less than truthful and dealing with insurance company.

Yes, came out okay, a bit banged up but it's a testament to the new car construction these days, the safety features work.

Update Oct 14, 2021
Just got an email from John, saying pretty much as follows.... (I could not help but post it verbatim. If we actually do get pictures tomorrow I would be happy as a squirrel in a bucket of acorns)

The Forum VI guitars are officially out of the queue and onto the start list. I should have some images for you folks tomorrow. We are rolling now. First stop, side-bending.
