Wayne Henderson has been building guitars in Virginia for 50 years. His waiting list is about 10 years long. But he is 77 years old now.
He runs a workshop and you can build a guitar in his shop in Marion, VA.

On a somewhat related note, I hired Wayne's daughter Jayne to do some custom inlay work for me a couple years ago on one of my electric guitars.  She does great work and builds some beautiful guitars under her own label.  I like how she focuses on using local woods and materials in her builds.  Cool stuff! I can only imagine such a day job.
